Tobacco Use and Help with Quitting Among Adolescents

Description: The percentage of adolescents who are screened for tobacco use and offered help with quitting if identified as a tobacco user.

Measure ID: 402

Type: Process

CMS Derm Specialty Set: Yes

High priority: No

Topped out: Yes

Reporting methods: Registry/QCDR

Maximum points: 10

Measure purpose: This measure encourages physicians to screen adolescents for tobacco use and offer tobacco cessation intervention when necessary. Research shows that people who begin smoking early in life are at greater risk for chronic tobacco use and associated health problems. Early identification of tobacco use and intervention can improve patient outcomes.

The numerator: The number of adolescent patients who were screened for tobacco use and who received tobacco cessation counseling intervention if identified as a tobacco user.

The denominator: All patients aged 12 to 20 who had at least one primary care visit during the reporting period.

Measure calculation example

Dr. Adams saw 113 adolescent patients during the reporting period. This is the denominator.

Dr. Adams screened 77 of these patients for tobacco use, offering tobacco cessation intervention to those who are identified as tobacco users. This is the numerator.

Quality of patient care = 77/113. Dr. Adams has a score of 68.1% for this measure. This translates to 3 points.

Important note

Because this measure is topped out, there is a gap between performance scores and the number of points earned. A physician who reports on only one patient encounter earns 3 points. A score of 99.99% earns 8.9 points, while physicians must score a perfect 100% to earn the full 10 points available.
