Measure purpose: This measure encourages physicians to provide coordinated care to patients who have a new occurrence of melanoma.

Calculation

The numerator: The number of patients with a new diagnosis of melanoma where a treatment plan is communicated to physicians providing continuing care within one month of the melanoma diagnosis.

The denominator: All patients diagnosed with a new occurrence of melanoma.

Measure calculation example

Dr. Zhao saw 100 patients who were diagnosed with melanoma during the reporting period. This is the denominator. 

Three of these patients ask Dr. Zhao not to communicate their treatment plan to their PCP. These patients are removed from the denominator because they meet exclusions for this measure.

Of the remaining 97 patients, Dr. Zhao communicated the patient treatment plan to the patient’s PCP or the referring physician 67 times. This is the numerator.

Quality of patient care = 67/97. Dr. Zhao has a score of 69.1% for this measure. This translates to 4.0 to 4.9 points. 

Important note

Because of how this measure is structured, physicians score 3 points even if they report on only one patient. As a result, physicians receive no additional score for reporting scores that fall below 49.44%. She would need to score more than 49% to earn additional points.
