Certificate of Attendance

To request a Certificate of Attendance for the 2009 Summer Academy Meeting click here to send an email request. The Subject line of the email should be listed as Certificate of Attendance. You will need to include your name as it appears on your registration record, along with the name of the meeting that you are requesting a Certificate for.

Certificates of Attendance will be sent out via the email provided on your registration record, if an email address is not available, the Certificate will be sent via fax.

Allow 5-7 business days for processing.
